2014 All American Scholastics Form
1 copy of your birth certificate or other accepted proof of age (such as a passport).
1 complete Pop Warner Medical Release Form for the player's physical exam (in calendar 2014) signed by a doctor, certifying that the player is physically fit to play tackle football. Do not wait on this one, set up the physical early.
1 complete Player Contract signed by the player and parent/guardian.
1 photograph, head and shoulders, NO HAT, 2"x2" or 2"x3".
2 copies of school report cards for ALL 2013-2014 school year grading periods. Must have all quarters, trimesters or semesters.
All players will be required to attend a conference certification day in late
August. This is mandatory for all 7,000+ players in the Peninsula Conference.
